February is Heart Month, a time to raise awareness about heart disease, prevention, and the importance of heart health. In honor of Heart Month, we’re highlighting the significance of Cardiac Rehabilitation and introducing our visiting Cardiologists who help patients recover and maintain heart health.
The Cardiac Rehab program, offered by CCHS Cardiac Rehab RN, Kate Deines, helps individuals recover after a cardiac event or procedure by promoting heart-healthy behaviors. The program includes structured exercise with individualized plans, blood pressure monitoring and telemetry, nutritional counseling and psychological support.
Whether you’re needing the Phase II Program for sessions after specific cardiac events/surgeries or are interested in our Phase III/General Prevention Program, Kate is here to support your journey to a healthier heart.

Recovering from a heart event or managing heart disease often requires ongoing specialist care to ensure long-term health and prevention. That’s why we are proud to offer access to four highly skilled visiting cardiologists who bring specialized cardiac expertise to Cozad Community Health System. These specialists diagnose, treat, and manage a range of heart conditions, including coronary artery disease, heart failure, arrhythmias, and hypertension. They provide preventative care, follow-up visits for post-cardiac events, and consultation on heart disease management.
