Transitional Skilled Care (Swing Bed)

If you or your loved one need short-term, skilled health care services, Cozad Community Hospital’s Transitional Skilled Care (sometimes referred to as Swing Bed) program is here to help. The program is designed for qualifying patients who are discharged from the hospital yet have a continued need for medical care.

The goal of the Transitional Skilled Care program is to provide you with the best therapy and nursing care in the shortest time possible. We want to help you feel better so you can return to your permanent living situation as soon as possible.

Cozad Community Hospital's Transitional Skilled Care program is designed to provide a quiet and peaceful environment, allowing you to get the rest you need so you can recover and return home.

Patients admitted to the Transitional Skilled Care program usually stay an average of 10 days or less.

For patients that require additional support to ensure a safe and comfortable hospital experience. Our facility is equipped with:

  • Overhead Lifts for Safer Mobility: Our patient rooms feature overhead lift systems that assist with safe repositioning and transfers, reducing strain on patients and caregivers.
  • Bariatric Beds & Seating: Our rooms are outfitted with bariatric-specific beds and chairs designed for enhanced stability, comfort, and support.
  • Specialized Wheelchairs & Walkers: We provide durable, high-weight-capacity mobility equipment to encourage movement and rehabilitation safely.

The Swing Bed program is available to qualifying patients — typically those who have completed a three-night inpatient stay. These patients no longer qualify for acute care services but are often not ready to return home.

The program provides care for patients who may have undergone surgery, experienced trauma, had an acute illness, require a wound vac, or need extended intravenous (IV) therapy.

Patients may also qualify for the program because of a new diagnosis or medical condition that requires additional education.

Medicare, Medicare Advantage, and additional health insurance plans are accepted on a case-by-case basis.
