Cozad Community Medical Clinic is excited to partner with Cozad Community Schools to offer an on campus Flu Shot Clinic for students and staff this fall. 

We are proud to make flu protection simple and convenient for our community. The Flu Clinic will be held on Wednesday, October 29th during the school day. 

To participate, please complete and return the consent form along with a copy of your insurance card to School Nurse, Sera Shoemaker, by Wednesday, October 22nd. If you have no health insurance or are underinsured, please indicate so on the form, vaccinations can still be made available to your children or uninsured staff. 

Our clinic staff will be on site to administer vaccinations, making it easy for your child or school staff member to stay protected throughout flu season.
